CRNA: RN Requirement Not Met

In your application for RN licensure with the CRNA, you attested to meeting the ___ requirement. In our phone discussion today, you confirmed you do not meet ___ requirement.

Therefore, the CRNA considers your RN permit was approved in error based on your false attestation. You have the following options:

1. Request the Registrar remove your name from the registered nurse register (HPA 35(2)) since your name was added in error.

• If you choose this option, your name will be removed from the register immediately and you will no longer be licensed as an RN in Alberta.
• You may re-apply for licensure as an RN in Alberta once you meet all requirements.

2. Request that the Registration Committee decide whether to direct the Registrar to remove your name from the register (HPA35(1)) since your name was added in error.

• The Registration Committee meets every 6-8 weeks and is comprised of RNs, NPs, and public members from across Alberta.
• You will have the opportunity to provide evidence for the Registration Committee to consider.
• If the Registration Committee decides to recommend that your name be removed from the register, your permit fee will be refunded.
• You may reapply for licensure as an RN in Alberta once you meet all requirements.

3. The Registrar may forward a complaint to the Complaint's Director (HPA 56) if there are reasonable grounds to believe that the false attestation on your application for RN licensure constitutes unprofessional conduct.

• The Complaint's Director may request an investigation and direct that your permit is suspended while an investigation takes place.
• As the result of an investigation, your license may be permanently revoked, conditions/sanctions may be added to your license and/or you may enter into an agreement with ongoing compliance requirements.
• All regulators across Canada are given notification of conditions added and removed from a permit including suspension (HPA 119).
