CRNA: RN Requirement Not Met

In your application for RN licensure with the CRNA, you attested to meeting the ___ requirement. In our phone discussion today, you confirmed you do not meet ___ requirement.

Therefore, the CRNA considers your RN permit was approved in error based on your false attestation. You have the following options:

1. Request the Registrar remove your name from the registered nurse register (HPA 35(2)) since your name was added in error.


2. Request that the Registration Committee decide whether to direct the Registrar to remove your name from the register (HPA35(1)) since your name was added in error.


3. The Registrar may forward a complaint to the Complaint's Director (HPA 56) if there are reasonable grounds to believe that the false attestation on your application for RN licensure constitutes unprofessional conduct.

Please confirm which option you would like to proceed with by ___. If we do not hear from you by ___, we may forward a complaint to the Complaint's Director.
