CRNA Application: Currency of Practice Information Required

You have attested to not meeting the currency of practice requirement and you have requested to go forward with the review of your application.

Under the delegated authority of the Registration Committee, the CRNA would assess your application and make a decision on your request for a permit when the currency of practice requirement has not been met.

Next Steps:

To assist with our review, please provide the following documentation within one week:

• Letter of current RN employment from your employer or employer offer letter.
• Confirmation of your RN practice hours for the past 5 practice years
  • 2026 (October 1, 2025 - September 30, 2026)
  • 2025 (October 1, 2024 - September 30, 2025)
  • 2024 (October 1, 2023 - September 30, 2024)
  • 2023 (October 1, 2022 - September 30, 2023)
  • 2022 (October 1, 2021 - September 30, 2026)

Please note that failure to meet the above deadline may result in a decision being made on your application based on the limited information we have to date unless a request for extension is granted.

Our decision could be to approve, approve with conditions, or refuse your application for a RN permit.

You may continue to practice on your ____ RN practice permit until a decision is made on your submitted application even after September 30.
