Under delegated authority from the Registration Committee, the CRNA has reviewed your application and approved your practice permit with the following condition:

Reason for the condition

In your renewal application you attested yes to one or more eligibility questions and were emailed requesting clarification about your attestation. We have yet to receive a reply and, as stated in the previous email, have made a decision based on the information we have.

Meeting the condition

Please provide more information about your attestation. Refer to the previous email for more information and check your spam/junk folder if needed.

Right to Request a Review

A decision to defer, refuse registration or to impose conditions on a registration and practice permit is subject to review by the registration review committee. Should you wish to request a review of this decision, please follow the requirements that are provided in section 41 of the HPA (see attached “HPA Section 41 Review by Council”).
