CRNA: RN Authorized to Prescribe and Order Diagnostic Tests

Good day,

This is to confirm that {{RegistrantFirstName}} {{RegistrantLastName}} (Registration #{{RegistrationNumber}}) has been authorized by the College of Registered Nurses of Alberta (CRNA) for RN prescribing Schedule 1 drugs (except for controlled drugs and substances) and ordering diagnostic tests.

They have been informed they must complete the Alberta Health application form to obtain a Practitioner Identification Number (Prac ID) and use this Prac ID when ordering diagnostic tests.
