CRNA: Condition Reminder

This is a reminder that you have active conditions on your permit. We recommend that you review the approval email or letter you received from the CRNA which outlines your conditions, deadlines surrounding them, and how to clear them.

If you have met these conditions or expect to meet them soon, please submit the required information. You have until the end of the practice year to meet the conditions, but submitting the information sooner will reduce delays with the completion of your renewal application.

If you expect you will not be able to meet your conditions by September 30, please let us know at your earliest convenience and we will confirm your next steps.
